Since writing the last comment I was contacted by phone by someone from Amazon who was in fact very helpful and managed to resolve all the outstanding issues we have with FBA, so credit to them for that.

Sounds great, but I am experiencing such a poor level of seller support at the moment that I can't imagine trying to set anything up with FBA other than doing the most simple of things. Your support infrastructure is simply not up to the job of encouraging new businesses to use FBA. Shame.
